When
you arrive for your first appointment, please allow
enough time to complete the new patient information
form, if you have not already done so.
It is also important to remember your prescription from
a physician, your insurance card or policy information,
and a current legal identification.
In order to serve you best, the average new patient
visit can last up to one hour. It is important that
we are able to devote the appropriate time to your care.
This gives your practitioner the time needed to get
to know you and respond to any questions you may have.
Please notify us at least 24 hours in advance if you
are unable to keep your appointment so that we can make
ourselves available to other patients that are also
in need of care.
Your practitioner will talk with you to learn your needs,
expectations and lifestyle goals as well as complete
an evaluation. You will undoubtedly have many questions
and may find it helpful to write out your questions
ahead of time in order to avoid forgetting important
items that you would like to discuss.
Some procedures require measurements and casting, so
you may want to wear shorts or other loose, comfortable
clothing. Gowns are also available upon your request.
